Why your AI agent needs a failure protocol

An agent that keeps trying the same broken fix costs you tokens and time. Left alone it will grind for an hour on a wrong hypothesis.

Mine now has a three strike rule. First failure, fix and recheck. Second failure, stop and rethink. Third failure, revert and document.
