For each repo the tracker sends one HEAD request per path on the default branch and records only whether the file exists and its size. It never downloads or republishes file contents. Files in nested folders (for example inside a sub-package) are not checked, and neither are other branches. Presence of a file says nothing about what is in it or how a project works. The sample is the top repos by stars per language, so it is not all of GitHub, and the number of repos per language differs slightly.
