An agent gives the right answer. During the same execution, a tool fails, a policy blocks a step, or a downstream action still occurs. Rereading the conversation will not reconstruct that path.

𝗩𝗜𝗦𝗜𝗕𝗟𝗘 𝗢𝗨𝗧𝗣𝗨𝗧 ≠ 𝗥𝗨𝗡𝗧𝗜𝗠𝗘 𝗦𝗧𝗔𝗧𝗘.

Microsoft warns that chat logs can omit tool actions, scopes and downstream authorization decisions. Production AI needs the familiar disciplines of logs, metrics, traces, events, dependency signals and exceptions but a dashboard showing activity is not an operating model.

The architectural question is: can teams correlate the system that produced the answer? Across an execution, ten layers merit inspection where applicable:

• IDENTITY and ORCHESTRATION: who initiated the work, under what authority, and which agent, instructions, workflow or sub-agent path ran?
• KNOWLEDGE and MODEL: what retrieval and grounding were involved, and which inference activity, provider or version is exposed?
• TOOL / ACTION, POLICY / CONTROL and DEPENDENCY: which calls and downstream actions ran, failed or were blocked, and which services participated?
• HUMAN, OUTCOME and CORRELATION: who approved or intervened; what completed, retried or timed out; and which trace, session or conversation identifiers connect the evidence?

Invocation volume, latency, tool failures, policy blocks and resource use can indicate where to investigate if instrumented. Evaluations of answer quality and business outcomes require different evidence. Correlated spans and events support investigation; they do not automatically prove causation.

Microsoft’s primitives offer partial views. Copilot Studio provides analytics and configured Application Insights exports; its OpenTelemetry-aligned environment telemetry is in preview and excludes declarative agents.
